Making Decisions Regarding Treatment and Recovery
The Decision Support Center (DSC) is a certified peer support program designed to empower individuals to take an active role in their recovery. We support clients who are involved in: medication management services, outpatient therapy, and substance use treatment (Recovery and Healing).
Our goal as part of their treatment team is to help individuals feel informed, confident, and heard.
Using CommonGround®
The DSC uses a web-based tool called CommonGround® to support shared decision-making between individuals and their medical providers.
With CommonGround®, individuals can:
- Complete a Health Report before appointments with their psychiatrist or CRNP
- Communicate their needs, preferences, symptoms, and goals more clearly
- Participate in collaborative and personalized treatment planning
CommonGround® also provides information about treatment options, practical tools and suggestions to help manage symptoms, and support for maintaining stability and wellness in daily life.
What Can You Do in the Decision Support Center?
When you visit the DSC, you can:
- Learn about your medications and how to use them safely and effectively
- Better understand your diagnosis so you can actively manage your health
- Discover your Personal Medicines—wellness strategies that help reduce symptoms and support recovery
- Create a Power Statement, a self‑advocacy goal that helps you communicate what matters most in your treatment
- Complete a CommonGround® Health Report to prepare for medical appointments
- Talk with peer support staff before or after your therapy or medical appointment
- Use the CommonGround® Library to explore recovery tools, education, and resources
- Participate in shared decision‑making, ensuring your voice is included as part of the treatment team
- Develop collaborative goals with providers to guide your next visit and ongoing care
